• This is a dental discount plan and is NOT dental insurance
• It CANNOT be combined with any other dental insurance
• This program is only accepted at Modern Smile Dental. Therefore, if you are referred to a specialist, they WILL NOT offer this discount.
• Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, auto, disability, or workman's comp type insurance are involved, this discount plan cannot be used.
• If you chose to extend your payment by paying through CareCredit for treatment, the restorative treatment discount is reduced to 5% due to merchant fees.
• You cannot use CareCredit to buy the plan.
• This offer cannot be combined with any other offer.
• Plan fees are only valid when paid at the time of enrollment.
